For your convenience, we offer our patient forms online. Please click here to print and fill out our patient forms prior to your visit. Once forms are complete, please mail, fax, or bring them to your next appointment!
Insurance information
Our treatment recommendations are based on what we decide mutually as the ideal treatment using the best materials and newest techniques. And we feel strongly that your insurance should not dictate your recommended treatment.

We take pride in our exceptional patient care along with delivering the best dental treatment options available to help you reach your goals. It is our continued commitment to provide the highest quality of dental care available to all our patients and to have those services comfortable affordable.
1st Visit:
Every new patient is given at least a 1-hour exam in order to fully understand what their questions, and concerns are, and also to get a full evaluation of their mouth. This exam includes:
- Soft Tissue Exam
- Oral Cancer Exam
- Digital X-Rays
- Periodontal Exam for Gum Disease
- Diagnostic cast/model of your teeth
- Series of Digital Photographs of your teeth
2nd Visit:
- View a digital tour of your mouth
- Dr. and Patient formulate a treatment plan that is appropriate for the patient’s needs and desires.
We will, as a courtesy, process your insurance benefits in our office. WE are happy to maximize your benefits by submitting the appropriate claim information and we will work with you to find a payment option that fits within your budget.
That is why our Patient Care Coordinator, Lori, has come up with some helpful information to assist in deciding which form of payment will work best for you and your individual needs. Whether you have been saving for a while, plan on using a low-interest credit card or just need some direction in choosing the finance company that will fit your situation, we are here to help. We are pleased to offer these payment options to assist you:

- Check
- Credit Card (VISA, MasterCard)
- Debit Card
If any of the above forms of payment are not options for you at this time, we also offer payment plans with Care Credit, an outside company. Care Credit specializes in dental procedures and creating a plan for longer-term payments.
Out of Town Guests
For our out of town patients, we offer car service to and from the airport and also to and from your dental visits. We also will arrange a stay at the award winning River Town Inn Bed and Breakfast in beautiful, historic Stillwater, Minnesota.
